Artist Form

  • Please fill out the form below with information about your artwork by April 22nd.

  • If you have more than one piece, please submit separate forms for each piece.

  • Please deliver your piece(s) to Mendocino Art Center no later than 3:30pm May 9th.

  • All wall pieces must be delivered ready to be hung.

One more request: If you have not already, please email us photos of your work if you haven’t already so we can begin to plan the layout of the show.
